Family doctors are the forefront of health care in Ontario. It is important to have a reliable primary care provider to work as your partner in maintaining your health and well-being. Family doctors treat a diversity of medical issues to meet the needs of children, teens, adults and elderly family members. Not only do family doctors perform a yearly check-up, they can also help you manage chronic conditions such as heart disease, diabetes and arthritis. They also treat acute situations such as sinus infections and injuries. When you need to see a specialist for any other serious concerns, a family doctor can help find someone who fits your specific needs.

What type of appointment do I need?

  • Meet & Greet

    This is the INITIAL appointment to meet your new Family Doctor. This appointment involves the following:

    1) An initial visit at 1CLINIC
    2) Thorough review & explanation of the Orientation Manual
    3) Not intended to discuss any NEW concerns or Comprehensive Annual Health Assessment (please book a separate appointment for this)

    At your appointment please ensure that you have the following:

    1) A VALID OHIP HEALTH CARD
    2) COMPLETED Patient Registration Form & Electronic Communications Consent Form

    We take your healthcare concerns seriously, so please be realistic about what we can accomplish in one office visit, as we always want to address your problems thoroughly. We strongly advise when booking appointments to only address ONE concern at a time to ensure your needs are acknowledged in a timely manner and investigated meticulously.

    Please be aware that NO SHOW FEES will be implemented if an appointment has not been cancelled within 24 BUSINESS HOURS.

  • Re-Orientation

    This is the INITIAL appointment for all of Dr. Linda Nguyen's rostered patients which involves the following:

    1) An initial visit at her new clinic-1Clinic
    2) Through review and explanation of The Patient Orientation Manual
    3) May follow-up on a previous investigation
    4) Not intended to discuss any NEW concerns or Comprehensive Annual Health Assessment (please book a separate appointment for this)

    At your appointment please ensure that you have the following:

    1) A VALID OHIP HEALTH CARD
    2) Patient Registration Form & Electronic Communications Consent Form

    We take your healthcare concerns seriously, so please be realistic about what we can accomplish in one office visit, as we always want to address your problems thoroughly. We strongly advise when booking appointments to only address ONE concern at a time to ensure your needs are acknowledged in a timely manner and investigated meticulously.

    Please be aware that NO SHOW FEES will be implemented if an appointment has not been cancelled within 24 BUSINESS HOURS.

  • Counselling

    Please ONLY book this appointment once you have seen your Family Doctor for a MEET & GREET appointment at 1Clinic.

    This appointment is intended for the following:

    1) Follow-up on the results of your Comprehensive Annual Health Assessment
    2) Follow-up on investigations that require thorough explanation
    3) Focused health concern that require a detailed discussion and examination (ie-mental health, chest pain, dizziness, etc.)

    At your appointment please ensure that you have the following:

    1) A VALID OHIP HEALTH CARD
    2) COMPLETED Patient Registration Form & Electronic Communications Consent Form

    We take your healthcare concerns seriously, so please be realistic about what we can accomplish in one office visit, as we always want to address your problems thoroughly. We strongly advise when booking appointments to only address ONE concern at a time to ensure your needs are acknowledged in a timely manner and investigated meticulously.

  • Follow Up

    Please ONLY book this appointment one you have seen your Family Doctor for a MEET & GREET appointment at 1Clinic. This appointment is intended for the following:

    1) Refill long-term medications (ie. birth control, blood pressure medications, etc.)
    2) Follow-up on results of one investigation that does not require thorough discussion (ie. thyroid blood work, urine culture test, etc.)

    At your appointment please ensure that you have the following:

    1) A VALID OHIP HEALTH CARD
    2) COMPLETED Patient Registration Form & Electronic Communications Consent Form

We strongly advise when booking appointments to only address ONE concern at a time to ensure your needs are acknowledged in a timely manner and investigated meticulously.

  • Comprehensive Annual Health Assessment

    Please ONLY book this appointment once you have seen your Family Doctor for a MEET & GREET appointment at 1Clinic. 1Clinic's primary focus is preventative care, hence a comprehensive annual health assessments are mandatory for rostered patients. When booking exams, patients should be prepared for the following:

    1) Thorough head to toe assessment
    2) Urine analysis
    3) Fasting blood work (By Appointment Only)
    4) Appropriate screening assessments

    At your appointment please ensure that you have the following:

    1) A VALID OHIP HEALTH CARD
    2) COMPLETED Patient Registration Form & Electronic Communications Consent Form
